Faceless Butcher first appeared in the Torment set in 2002, representing the dark, atmospheric horror aesthetic central to the plane of Otaria. The card was designed to provide black-aligned players with a creature-based removal effect, reflecting a period in Magic: The Gathering history where designers frequently experimented with 'O-Ring' style mechanics placed on creature bodies. Its return in the 2023 Dominaria Remastered set served as a nostalgic tribute to the card's original utility in Limited formats, where it functioned as both a recurring threat and a disruptive tool for board control.

The original illustration by Arnie Swekel leans into the grotesque nature of the creature, depicting a masked, multi-limbed entity that embodies the visceral fear associated with the Cabal, a powerful shadow organization within the game's lore.
