Failed Inspection was released in 2016 as part of the Kaladesh expansion for Magic: The Gathering. Set on the plane of Kaladesh, a world defined by its focus on invention and intricate artifice, the card serves as a thematic reflection of the setting's strict regulatory atmosphere. It captures a moment where the Consulate, the governing body of the plane, rejects a piece of machinery for failing to meet their rigid safety or legality standards.

The card’s illustration was created by artist Matt Stewart, who sought to visually communicate the tension between the inventor and the authority. Stewart utilized a clean, structured aesthetic to match the architectural style of Kaladesh, emphasizing the contrast between the vibrant, creative spark of the invention and the cold, institutional judgment imposed by the inspector.
