Under Prison, illustrated by Miyao Ikumi, emerged from the dark, psychological landscape of seinen manga during the early 2000s. The series was crafted to explore the intense, visceral confinement of its titular setting, blending disturbing imagery with a high-contrast aesthetic that became a hallmark of Ikumi's portfolio. It serves as a stylistic artifact of a period when extreme, surreal horror was finding a dedicated niche in Japanese monthly serializations.

Ikumi's design approach relies heavily on heavy ink work and distorted anatomical proportions to heighten the reader's sense of unease. The artistic intent was to move beyond conventional storytelling and prioritize atmosphere and dread, resulting in a work that is frequently discussed for its bold, unconventional panel layouts and uncompromising, gruesome visual themes.
