Boneheimer was released as part of the Labyrinth of Nightmare expansion, which debuted in the North American TCG market in 2003. This set followed the established pattern of adapting Japanese-exclusive OCG releases, bringing iconic early-era monsters and trap cards to the western game. As a standard normal monster, Boneheimer functioned as a fundamental deck-filler during the formative years of the Yu-Gi-Oh! Trading Card Game, characterized by the simple, dark fantasy aesthetic that defined the initial creative direction of the franchise.

The card was illustrated with a focus on macabre, skeletal imagery, consistent with the thematic naming conventions of the Labyrinth of Nightmare set, which prioritized supernatural and gothic motifs. The design was produced by Konami’s internal art team, aiming to flesh out the growing card pool with low-level monsters that offered basic gameplay utility while contributing to the cohesive, gloomy atmosphere of the game's second generation of expansion sets.
