Necroface was first introduced in the Yu-Gi-Oh! Trading Card Game as part of the 2007 Duelist Pack: Rivals of the Pharaoh, a set designed to showcase cards utilized by the primary antagonists and rivals of the original series. The card is based on the darker, spectral aesthetic associated with the character Bakura, whose deck themes frequently focused on the manipulation of the Graveyard and the banishing mechanic. By forcing both players to banish cards from their respective decks, Necroface serves as a disruptive engine designed to accelerate the depletion of resources.

The design of the monster reflects a macabre, surrealist influence common in early Yu-Gi-Oh! card art, emphasizing a grotesque, multi-faced entity.
