Geier Reach Sanitarium was first introduced in the 2016 expansion Eldritch Moon, later appearing as a reprint in the Commander 2019 set. Within the narrative of the Magic: The Gathering multiverse, the location is a mental institution situated in the province of Stensia on the plane of Innistrad. The design reflects the gothic horror themes prevalent in the setting, illustrating a place where the desperate seek refuge from the supernatural terrors plaguing the world, only to find the line between sanity and madness blurred by the influence of cosmic entities.

Artist Cliff Childs provided the illustration for this land card, utilizing a muted, atmospheric color palette that emphasizes the gloom of the Innistrad landscape. The design aims to capture the architectural style of a Victorian-era asylum, creating a sense of foreboding isolation.
