Necropolis Fiend was originally printed as a rare creature card in the 2014 Khans of Tarkir expansion, later appearing as a promotional inclusion in the Fate Reforged Clash Pack released in early 2015. The card was designed to highlight the graveyard-centric mechanics of the Sultai Brood, a faction within the Tarkir setting defined by their use of black, green, and blue mana to exploit the dead for tactical advantage.

The artwork was illustrated by Aleksi Briclot, a frequent collaborator on the Magic: The Gathering franchise. Briclot focused on capturing the monstrous, decayed aesthetic of the Sultai, emphasizing the creature's massive scale and necrotic essence. Upon its release, the card was noted for its delve ability, which allowed players to exile cards from their graveyard to reduce its casting cost, a mechanic that became a centerpiece of competitive play during that era.
