Painwracker Oni was released in 2004 as part of the Champions of Kamigawa set, the first installment in a block heavily inspired by Japanese mythology and folklore. The design team sought to capture the aesthetic of 'oni'—malevolent, horned demons from traditional legend—by integrating them into the black-aligned discard and sacrifice mechanics central to the set's identity. These creatures were intended to represent the physical manifestation of the spirit world's corruption during the conflict depicted in the block's narrative.

The card’s illustration was crafted by artist Ron Spears, who utilized a stark, aggressive visual style to emphasize the creature's menacing nature. Within the competitive landscape of the time, the card was received as a niche tactical piece, primarily utilized in limited formats where its requirement for a sacrifice could be mitigated by recurring cheaper creatures.
