Pestilence was first introduced in the Alpha set of Magic: The Gathering in 1993, emerging from the game's earliest design phase where black-aligned cards were defined by high-risk, high-reward mechanics that often demanded life payments. The card reflects Richard Garfield’s initial vision of a game where resources, specifically life totals, were intended to be volatile and constantly shifting. The Sixth Edition printing, released in 1999, served as a core set update that maintained the card's mechanical identity while introducing updated frame aesthetics.

The artwork for this version was commissioned from Kev Walker, a prolific illustrator whose style became synonymous with the grittier, more visceral aesthetic of Magic in the late 1990s. Walker’s depiction of Pestilence leans into the card's destructive nature, favoring a dark, atmospheric composition that captures the creeping dread of the enchantment. Upon its release in the Sixth Edition, the art was well-received by players for its ability to convey the card's function—an inevitability of decay—without relying on overly literal imagery.
