Plague Engineer was first released in the 2019 expansion Modern Horizons, a set specifically designed to introduce powerful, cards directly into the Modern format without requiring them to pass through a Standard legality cycle. The card was conceived as a disruptive tool for black-aligned control and midrange decks, intended to punish strategies that rely on wide boards of low-toughness creatures, such as tribal decks or token strategies.

The art was rendered by Nicholas Gregory, whose work for the set captures the macabre and industrial aesthetic of the Plague Engineer. Gregory utilized a dark, visceral color palette to emphasize the character’s role as a harbinger of decay, effectively communicating the card’s mechanical identity through its visual presence.
