Mysterious Girlfriend X originated as a serialized manga written and illustrated by Riichi Ueshiba, beginning its run in Kodansha's Afternoon magazine in 2006. The series is celebrated for its highly distinctive, retro-inflected art style that pays homage to late 20th-century manga aesthetics, characterized by Ueshiba's use of sharp lines and moody, atmospheric paneling. It was conceived as a surreal exploration of teenage intimacy, centered on the peculiar telepathic bond formed between high school student Akira Tsubaki and the enigmatic transfer student Mikoto Urabe through the sharing of saliva.

Upon its release, the work was noted for its unconventional approach to the romance genre, prioritizing psychological tension and bizarre internal logic over standard shonen tropes. The visual design remains one of the series' most defining features, with Ueshiba employing a technique that feels both nostalgic and unsettling, effectively grounding the story's supernatural premise in a grounded, everyday high school setting. It eventually inspired a 2012 anime adaptation by studio Hoods Entertainment, which sought to replicate the manga’s specific visual quirks and slow-burn pacing.
