A young Taiwanese assassin codenamed "Glass Heart" committed suicide by jumping off a building, and her heart was pierced by metal fence. Miraculously, her life was saved by heart transplantation. During her recovery she began to experience strange dreams, which lead her to Japan looking for the donor of her heart, who happens to be Kaori Makimura, former partner of City Hunter Ryo Saeba.
The author mentioned in the first tankoubon volume that Angel Heart only shares the same characters of City Hunter but not it's continuation. The events are all parallel universe, therefore it has nothing to do with City Hunter.

Where it came from
Angel Heart, a 50-episode television series produced by TMS Entertainment and broadcast in 2005, serves as an alternate-universe sequel to the seminal manga City Hunter. Created by Tsukasa Hojo, the series shifts the tone from the high-octane comedy of its predecessor to a more melancholic, reflective exploration of grief and redemption. The narrative follows a professional assassin known as the Glass Heart who receives a heart transplant from the deceased Kaori Makimura, leading her to seek out Kaori's former partner, Ryo Saeba, in the streets of Shinjuku.
The production sought to balance the familiar iconography of Hojo's character designs with a more mature, dramatic visual language befitting its themes of found family. While the adaptation faced scrutiny from longtime fans of City Hunter due to its departure from the established canon and lighter narrative roots, it was praised for its production quality and its ambitious attempt to evolve the franchise into a character-driven drama. The series remains a notable example of late-2000s anime experimentation with tone and franchise legacy.
