Island Sanctuary was originally released in the Alpha set of Magic: The Gathering in 1993, designed by Richard Garfield to represent a defensive position that limits both the player and their opponents. It was later included in the Fourth Edition core set, which was released in 1995 as the first large-scale reprint expansion intended to stabilize the game's card availability during its initial growth phase. This edition featured a modernized card frame design that replaced the original Alpha/Beta aesthetic, marking a significant transition in the game's visual identity.

The artwork for this specific version of the card was created by Mark Poole, one of the original illustrators whose work defined the early visual language of the game. Poole’s contribution to Island Sanctuary focuses on a serene, mystical depiction of a protected coastal haven, utilizing his signature style of soft, ethereal tones.
